Harm Reduction Services

We provide syringe exchange, safer drug use supply provision, education, and counseling which provide people who use drugs with the tools they need to reduce the potential harms associated with drug use and needle-sharing, including information on safer sex, HIV/AIDS prevention, and treatment. We do this through the following:

Outreach and Public Safety Team (OPST)

The Outreach and Public Safety Team (OPST) service East Harlem, Washington Heights, and the South Bronx to conduct outreach to community members in these neighborhoods, divert public use, clean up syringe litter in parks, playgrounds, and community centers. The OPSTs utilize our mobile outreach vans to distribute supplies, including safe injection kits, safe sex kits (including PrEP resources and condoms), Naloxone kits, substance test kits (including testing strips for fentanyl and other synthetic drugs), safe smoking kits, and wound care management supplies.

Staff also conduct Hep-C and HIV rapid tests and make referrals to supportive services, as needed. The mobile vans provide services similar to those offered through our Drop-In Centers at our brick-and-mortar locations, including distributing safe supplies, testing, linkage to care, treatment and detox referrals, and other supportive services.

Peer Delivered Syringe Exchange (PDSE)

The PDSE team is OnPoint NYC’s boots-on-the-ground outreach team with roving staff who work in high-need neighborhoods where there are very few other service providers available in upper Manhattan and the South Bronx to bring more services to these locations.

Syringe Kiosks

We partner with the NYC Parks Department and the MTA to install and manage hazardous waste containers for community members to deposit used syringes in parks, playgrounds, and near transportation hubs. Staff also provide education to community members who use injection drugs throughout upper Manhattan and the South Bronx to show them where the kiosks are located and how to properly dispose of their used syringes to reduce public syringe litter on the streets and in parks.

Syringe Services Program Waivered Sites

Our Syringe Services Program (SSP) waivered sites are the oldest part of our program.  The Syringe Exchange Program (SEP) waiver provides us with NYS Department of Health (DOH) authorization to give out syringes at specific locations in New York City, where we can remain for up to five hours at any given time. Our SSP waivered sites are located in places where there is a documented history of injection drug use already taking place. Our waivers were granted with community involvement and support. We distribute syringes, Naloxone kits, hygiene kits, safe-sex kits, wound care kits, substance test strips, including test strips for fentanyl, and other harm reduction supplies. 

We also have a Case Manager or Testing Navigator there to conduct HIV and Hep-C testing and linkage to care. Our longest-standing location is at 110th Street & Park Avenue in East Harlem, where we have had an SSP waiver since 1992. We have other locations in upper Manhattan and the South Bronx, and we are set up at these locations Tuesday-Friday.

Other Services

Our Drop-In Centers (DIC) are no-barrier, walk-in spaces at both locations, East Harlem and Washington Heights, where we offer coffee, snacks, showers, bathrooms, and laundry. We focus on meeting participants’ basic needs, building trusting relationships, and then gradually engaging individuals in higher levels of care.

We provide syringe exchange, safer drug use supply provision, education, and counseling which provide people who use drugs with the tools they need to reduce the potential harms associated with drug use and needle-sharing, including information on safer sex, HIV/AIDS prevention and treatment.

We made history and became the first organization in the US to open two OPCs (also known as Supervised Consumption Centers) that monitor drug consumption on-site in East Harlem and Washington Heights. The program reduces overdose death and health risks associated with drug use.

We offer an array of in-house clinical services for people who use drugs or engage in sex work including HIV and Hep-C testing, wound care, Medication-Assisted Treatment (MAT), including buprenorphine, medical assessments, and counseling. We also provide the COVID-19 vaccine, flu shots, acupuncture, acupressure, and reiki.

We operate a hotline number (718-415-3708) for community members to report syringe litter or public drug use. Our staff dispatches a team to clean up the area and to help divert public use. We also educate our community members on harm reduction, overdose prevention, and other services.

We have teams of Case Managers at both of our locations who conduct benefits navigation, advocacy, health navigation, case conference sessions with various providers, drug education groups, individual counseling, and modalities for safer drug use.
