MAT Navigator – HUB

Position Summary

This position for MAT Navigator will work with the clinical unit and is responsible for providing navigation support, benefits navigation, appointment reminders and support participant’s individualized mental health and Buprenorphine services. The MAT Navigator will work alongside the Case Manager and Harm Reduction Mental Health Counselor to provide low threshold access to buprenorphine services to ONPOINT NYC participants and community members.

This position will work both in-house and via street-based outreach helping facilitate access to buprenorphine and individual counseling sessions as needed to address barriers with access to MAT and maintaining adherence. All activities are designed to be consistent with OnPoint NYC’s service model, which is based on educating and empowering individuals to advocate for themselves and make self-identified changes in their lives. The MAT Navigator will have experience with MAT and sustained recovery.

Pay Rate: Hourly:  $23.63 – $24.73
Position Type: Full Time
Annual Salary: $43,000 – $45,000
Hours Per Week:  Monday-Friday 8:30 am-4:30 pm

Responsibilities
  • Engage ONPOINT NYC participants and community members who are seeking Buprenorphine information and treatment access
  • Collaborate with ONPOINT NYC’s Drug User Health Hub to identify and address participant barriers with maintaining adherence with buprenorphine treatment as prescribed
  • Provide education and support to participants accessing MAT to encourage adherence with treatment as prescribed
  • Assist clinical staff to provide reminders for scheduled appointments, provide escort and minimize barriers with access to Buprenorphine treatment
  • Assist Buprenorphine Program with client engagement and tracking as assigned
  • Conduct community outreach events to promote Buprenorphine Program and services
  • Follow-up on referrals and linkages to providers
  • Assist with health insurance enrollment, changes, to maintain treatment adherence
  • Keep accurate, consistent, complete and intelligible records and data on all the project activities and participants served
  • Maintain a professional working relationship with the health care providers and community-based organizations providing services to participants, including the ONPOINT NYC Health HUB Clinic providers on-site.
  • Advocate for ONPOINT NYC participants and communities and help empower participants to advocate for themselves as well to receive the services that they need to be healthy.
  • Perform other related tasks and responsibilities as assigned.
 
Qualifications
  • Must have knowledge with MAT, specifically Buprenorphine and sustained recovery
  • Must be an organized, self-motivated, team-player, with strong interpersonal and communication skills, basic computer skills and the ability and willingness to engage in hands-on work and solve problems as they arise
  • Significant life experience in the areas of drug use, particularly OUD and access to MAT
  • Ability to work with diverse communities (multi-gender, -ethnic, -generation, -lingual, political) as well as enthusiasm working with individuals who are current substance users
  • Strong ability to promote navigation services and connect participants to other services, including mental health, MAT, and other supportive services
  • Must be familiar with and believe in a harm reduction philosophy
  • Working knowledge of Microsoft Word, Excel, Internet, and email correspondence
  • Must be willing to work outdoors and at street sites (sometimes in inclement weather).  Hours may include nights and weekends
