Our case managers are advocates and coaches in connecting participants to programs and services such as health insurance and income benefits, counseling and mental health services, community support and securing vital forms of identification and housing.
As a result of stigma, punishment, and isolation, our participants are routinely disconnected from people and services. Having the coaching and support to establish connection to vital services means participants spend less time navigating barriers and more time working toward wellness and stability.
